The Community Rehabilitation Centre provides team rehabilitation on an outpatient basis to people living in the community of all ages.
On referral to the centre a comprehensive initial assessment with the CRC Coordinator may be required to ensure all aspects of your care are considered.
Staff contributing to rehabilitation programs include:
- Physiotherapists
- Occupational Therapists
- Exercise Physiologists
- Nurses
- Dietitians
- Speech Pathologists
- Psychologists
- Allied Health Assistants
- Coordinators
- Pharmacists
- Specialist Medical Practitioners (Specialist Clinics)
Referrals
GP referrals are preferred (and required for some specialist clinics); however, anyone can make a referral to the service including health professionals, you or your carer. Waiting Times
All referrals to our services are assessed and prioritised according to your care needs and you will be contacted by the centre to advise if you have been waitlisted or allocated an appointment.
With your consent, referral acknowledgement and relevant correspondence will be sent to your GP to ensure continuity of care.
